Menu and screen resolution issues

Lockjaw	So I've managed to get it work with no-CD and the latest patch, but there are 2 things I can't handle. 1. The game menu is just a black screen. Can I do something to fix it? 2. I can't make the game window bigger than 640x480 resolution. Any tips to fix that as well? Thanks.
